The application for a Recreational Camp for Children, pursuant to 105 CMR 430.00, is required when any camp or program, as defined by 105 CMR 430.000, operates at the University of Massachusetts Amherst campus. The completed application, fees, attachments, and certifications are required to be submitted 90 days in advance of the camp opening date. If extenuating circumstances prevent a 90- day pre-submittal period, a written variance request may be submitted to the Assistant Director for Environmental Health Services at EH & S. In no cases, shall a variance be granted for less than a 30- dav submittal. In all cases, the camper's health records and immunization records, commensurate with 105 CMR 430.150-153 shall be made available to the Assistant Director for Environmental Health Services at least five business davs prior to the camp opening date. In all cases, the counselor's health records, immunization records, and certification of CORI/SORIs received, reviewed and approved by the Camp Director shall be made available to the Assistant Director for Environmental Health Services at least five business days prior to the camp opening date.
Reminder: Any youth programs not meeting the definition of a camp, and therefore not registering or applying as such, cannot engage in any high-risk activity such as swimming while they are on campus.
