In extenuating circumstances, for religious or cultural concerns when UMass Catering, (Auxiliary Enterprises) is unable to provide the service, a food waiver approval is required from UMass Catering (Auxiliary Enterprises) and is further reviewed for food safety from Environmental Health & Safety, (EH&S). This E-Sign form will automatically be forwarded to 1). Auxiliary Enterprises, 2). EH&S, and 3). The SORC for the Student Activities Office. You will receive an approval or denial of this food waiver through your email. If you need food safety training for an approved food waiver event, please contact the Assistant Director for Environmental Health, Alyssa Rusiecki, at 413-545-5110 at least 30 days prior to your event.
